New Ministers need to fill out the New Ministers form first, then fill out the Ministers Registration Form.

You need to fill out a form for each child you plan to take to Fairview for childcare.

Note – you can pay online through the Ministers Registration or the Wives Registration forms.  You can also choose to pay at the church.
